Volunteer Criminal Background Check, Application, and Release Form
Volunteers needing a background check may include the following:
- Junior Achievement Volunteers
- Volunteer Coaches or Club Leaders
- Classroom/Building Volunteer
- School/Class Event Volunteer
- Field Trip Volunteer
- Any activity sponsored by the school not mentioned above, but deemed to need a background check
Please read the form carefully, answer the questions, attach a photo ID and sign before submitting it to the appropriate building administrator.
