Volunteer Criminal Background Check, Application, and Release Form

Volunteers needing a background check may include the following:

• Junior Achievement Volunteers
• Volunteer Coaches or Club Leaders
• Classroom and/or Building Volunteer
• School and/or Class Event Volunteer
• Field Trip Volunteer
• Any activity sponsored by the school not mentioned above, but deemed to need a background check

Please read the form carefully, answer the questions, attach a photo ID and sign before submitting it to the appropriate building administrator.
